Metonitazene (or "Mitazuni" as a possible phonetic variant or street name) is an extremely powerful synthetic opioid drug. It belongs to the class of nitazenes (or benzimidazole opioids). It's not a direct copy of fentanyl (which is in the piperidine family), but it's often mixed with it on the illegal drug market to make it even deadlier. It was first developed in the 1950s as a painkiller, but it popped up as a new psychoactive substance (NPS) around 2020, leading to a scary rise in overdoses.
Key Features
Strength: It's estimated to be 10-20 times stronger than fentanyl, making it deadly even in tiny doses (less than 1 mg). An overdose can happen with an amount about the size of a few grains of salt.
Look and Use: It comes as a white or brown powder, often sold as fake heroin, ketamine, cocaine, or counterfeit pills. On the black market, people snort, inject, or swallow it.
Spread: It's been found in North America, Europe (including Italy), Australia, and other places. In Italy, health authorities are watching nitazenes closely because of the risk of contamination in illegal drug supplies, with more reports starting in 2023.
Risks and Dangers
Overdose: Symptoms include slowed breathing, confusion, extreme drowsiness, and sudden death. It takes multiple doses of naloxone (Narcan) to reverse it, unlike with fentanyl.
Health Impact: It's helped cause a spike in deaths from synthetic opioids. For example, in the US, it's been linked to hundreds of forensic cases since 2020.
Tips: If you think you've been exposed, call emergency services right away (118 in Italy). Opioid test kits and harm reduction programs (like those from Harm Reduction International) are key.
